I've created a first draft while I get acquainted with the different formats to submit a more compliant proposal, but I want to circulate this draft for comments.  I've only done three of the suggested headers and did not add the output for the examples—didn't find a suggested way to do it.

I tried to look at all the different "spots" that would have to be touched, but I may have missed something.
